We are delighted to have you as a participant in classes and workshops at Center of Gravity Yoga, LLC. As you know, exercise programs and events involve risk of injury. If you have any physical or medical conditions, or if you have any concerns regarding your ability or susceptibility to injury, please consult your physician before starting any new yoga or exercise program. By choosing to participate at Center of Gravity Yoga, LLC, you voluntarily agree to the conditions stated below. I am choosing to participate in the classes or workshops offered by Center of Gravity, LLC during which I will receive information, instruction, and an opportunity to move and exercise. I recognize that classes require physical exertion that may be strenuous and can cause physical injury, and I am fully aware of the risks and hazards involved. I understand that I take full responsibility for and assume any risks, injuries, or damages, known or unknown that I might incur as a result of participating. It is my responsibility to consult with a physician and follow the advice given. By signing this waiver, I and my heirs hereby release all owners, instructors, organizers, building owners, and vendors, and agree to fully indemnify them, for any and all claims, expenses, or liability for injuries that may directly or indirectly occur as a result of my participation in classes and workshops both today and in the future.
